Patient Care Coordination

This week, your readings focus on innovative solutions to improve patient care, including the patient-centered model of care. Consider what the nurse’s role is in this particular model. It certainly involves our profession collaborating with all health professions in caring for a population of patients with multiple needs and conditions. Nurses’ expert skills will be in high demand to take the lead concerning patient assessments, plans of care, education, evaluation, and outcomes of care.
